APD Waiver Services

As an approved APD provider, we deliver person-centered supports and nursing services designed to empower individuals with developmental disabilities to achieve their goals and maximize independence — throughout the State of Florida.

Our Approach

What Are APD Services?

The Agency for Persons with Disabilities (APD) administers the Home and Community-Based Medicaid Waiver program, which funds services that help individuals with developmental disabilities live, work, and thrive in their communities.

At Embracing Love with Care, we are an approved APD provider committed to delivering high-quality, individualized services — including nursing supports — that honor each person\u2019s dignity, choices, and goals. We work closely with APD Support Coordinators, families, and individuals to build meaningful support plans throughout the State of Florida.

What We Provide

APD Waiver Services We Offer

Our APD services are designed to support every aspect of an individual\u2019s life — from daily living and life skills to community participation and nursing supports.

Our Process

Person-Centered Goal Planning

We believe every individual deserves a support plan that reflects who they are — not just what they need. Our person-centered approach puts the individual at the center of every decision.

1

Discovery & Assessment

We begin by getting to know the individual — their strengths, interests, preferences, and support needs.

2

Person-Centered Plan

Together with the individual, family, and support coordinator, we develop a plan built around their unique goals.

3

Matched Support Team

We carefully match each individual with Direct Support Professionals who align with their personality and goals.

4

Ongoing Review & Growth

Plans are regularly reviewed and updated to reflect progress, changing needs, and evolving aspirations.
